The Diabetes Nurse Educator works within a primary health care delivery model that utilizes an integrated, interdisciplinary team of health professionals and community members in the delivery of diabetes prevention, screening, intervention, and education programs which support and improve the health of clients. Reporting to the Director of Programs & Community Services, the candidate will provide community based diabetes education services to the FHQ Tribal Council First Nations.
Responsibilities
Work closely with the Community Dietitians to coordinate and manage the implementation of the Aboriginal Diabetes Initiative
Integrate knowledge, skills and the principles of teaching and learning, in the provision of community-based diabetes care for clients, families, and community(s)
Monitor, evaluate and report results of coordinated, integrated care
Flexibility in responding to requests for chronic disease programs and presentations to address community needs
Works in collaboration with Pasikow Muskwa Rising Bear Healing Centre, All Nations Healing Hospital acute care unit, and Home and Community Care program to address client care needs related to diabetes and chronic illness.
Qualifications
RN (BScN)
Active registration with SRNA
Certified Diabetes Educator (preferred)
Experience working in Diabetes Education an asset
Knowledge, understanding, and cultural sensitivity working with First Nations.
Must possess a valid drivers license
