We are looking for a Full time Care & Housing Coordinator.

Main Activities
Work with people living with or at high risk for HIV/AIDS who may also be affected by mental illness, addictions, poverty, homeless and other psychosocial factors
Locate housing and provide coordination with residents to secure housing for transition out of Sanctum
Develop and maintain working relationships with housing services in Saskatoon
Work with patients on individual goals aimed at recovery and management of their illness(s)  
Provide therapeutic counselling/assistance to clients
Advocate for additional service and resources
Establish and maintain current, accurate, confidential files for each client
Assist clients during their move into permanent housing. Such assistance may include accessing rental assistance money, security deposit, donated furnishings, coordinating with movers, and coordinating transportation.
Assist clients on overcoming any barriers to housing and provide crisis intervention/prevention as need in order to maintain housing
Manage any unanticipated events or unstable situations
Qualifications
The successful candidates will have the following skills and experience
Knowledge of community resources with an emphasis on housing services
Maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
An understanding of the issues related to HIV including harm reduction
Conflict resolution and interpersonal skills
Background working in Mental Health and Addictions
Degree in social work, addictions or related field
