Job Description: This position reports to the PHC manager, the CDM Nurse Educator is responsible for assessing learning needs, developing, coordinating, implementing and evaluating self-management educational/counselling activities for clients, families, health professionals in one or more of the following areas: cardiovascular, diabetes, general respiratory disease etc by implementing accepted standards and guidelines. Preference given to certified diabetes educator. Rural travel within the South West is required.
Required Qualifications
Bachelor Degree/Diploma in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Valid drivers license and the ability to travel both within the Region and Provincially.
Experience
Minimum 3 years relevant Acute Care Nursing experience
Minimum 1 year supervisory experience
Baccalaureate degree in education/adult learning an asset.
Minimum of three years experience in chronic disease/complex illness
education or chronic disease management, especially diabetes, cardiac
and renal disease.
Experience in program development; experience in
community development an asset. Current BCLS certification,
demonstrated computer literacy (proficiency in Microsoft applications),
Certified or willingness to obtain nursing certifications as required.
Certified diabetes education certificate considered an asset
Knowledge, Skills and Abilities
Change management skills
Ability to competently use computers and software applications as required by the position
Other Information
Field hours
Some evening and weekend work may be required.
