Job Description
The HIV AIDS Response Team is a dynamic team that delivers rapid wrap around in- patient support to persons living with HIV. This support is meant to address and close existing gaps and barriers both medical and social in nature to allow for stabilization and improved health and social outcomes for persons living with HIV.
Work with people living with HIV/AIDS
The HIV AIDS Response Team is a dynamic team that delivers rapid wrap around in- patient support to persons living with HIV. This support is meant to address and close existing gaps and barriers both medical and social in nature to allow for stabilization and improved health and social outcomes for persons living with HIV.
Work with people living with HIV/AIDS who may also be affected by mental illness, addictions, poverty, and other psycho social factors
Coordination of psycho social and medical components of individual care plans, facilitating referrals to appropriate agencies which will assist in attaining goals of care plans, Locate housing and provide coordination with residents to secure housing upon discharge from hospital
Provide therapeutic counselling/assistance to clients who may present with multiple issues affecting their ability to complete treatment of acute illness,ensure outcomes are recorded, measured and documented in individual charts as per the direction of the Executive Director
Assist client with securing forms of identification, assist with legal issues, provide necessary transportation to appointments
Cultural sensitivity of First Nations issues
Extensive knowledge of community resources with an emphasis on life skills support, housing services and how to access and advocate for these services on behalf of clients
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds.
Ability to communicate with and maintain an effective working relationship with other members of team and health care providers
An understanding of the issues related to HIV including harm reduction
Conflict resolution and interpersonal skills
Background working in Mental Health and Addictions
Degree in social work, addictions or related field
Valid drivers license
Vehicle
