Hours: 35 hours per week
Location: Kamsack (4 days/week) and Yorkton (1 day/week)

The Positive Impact Program aims to support targeted clients to reduce barriers and potential harms by supporting access to basic needs, harm reduction materials and services, and social and community supports through referrals, information, and collaboration with Saskatchewan Health Authority HIV Strategy Coordinator.

The Positive Impact Program aims to support targeted clients to reduce barriers and potential harms by supporting access to basic needs, harm reduction materials and services, and social and community supports through referrals, information, and collaboration with Saskatchewan Health Authority HIV Strategy Coordinator.
SIGN is looking for a compassionate, client centred and energetic individual, with excellent communication, leadership and problem-solving skills. The successful candidate must be self-motivated, collaborative and able to work in varied settings, including the Opioid Agonist Therapy Clinics, community outreach and venues, and client homes.

Expectations:
Communicate effectively and professionally
Coordinate and participate in case management and consultation
Provide direct client access or referral to harm reduction services
Plan and facilitate community harm reduction education and events
Disseminate and promote education and resources on HIV, Hepatitis C and STIs
Attend meetings as required
Prepare reports, statistical information and assist in preparation of program budget
Ensure that safe work practices are followed
Maintain and model appropriate personal boundaries with others
Respect and protect the rights of clients
Qualifications and experience:
A Bachelor or Masters of Social Work Degree
Experience working with the harm reduction model and knowledge of STBIIs
Experience working with individuals experiencing chronic health and social issues
Valid Saskatchewan Drivers License and vehicle in good working order
A recent and clear Criminal Record Check and Vulnerable Sector Check from RCMP
