Job Description:
The CDM Clinical Nurse Educator reports to the Manager Clinical Integration and works with an interdisciplinary team with a chronic disease prevention and management focus. The CDM Clinical Nurse Educator combines in-depth clinical expertise, with the principles of teaching and learning, in the provision of self-care management for clients, families, health professionals and students in group and individual settings. This position is responsible for cardiac education and the coordination of a multi-condition CDM rehabilitation exercise program. The CDM Clinical Nurse Educator also supports other chronic disease management services and the exercise programs.

Experience
- 3 years clinical experience with patients/families with chronic disease
Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Valid Class 5 driver's license
Other Education and Training
- Basic Life Support Certificate (BLS)
- Nursing diploma is acceptable
