Job Description
Duties & Responsibilities
- Engage individuals with mental health, substance use struggles, houselessness, etc. Provide assistance in identifying and accessing pathways to healthier living.
- Assist with various social systems navigation.
- Promote cultural considerations through the system navigation process. Connect individuals with culturally relevant events, Elders, smudging, community groups etc.
- Conduct regular outreach in the community at strategic locations to identify high needs

- Assist with various social systems navigation.
- Promote cultural considerations through the system navigation process. Connect individuals with culturally relevant events, Elders, smudging, community groups etc.
- Conduct regular outreach in the community at strategic locations to identify high needs individuals. Provide outreach with a focus on prevention, diversion and healing.
- Work with outside government agencies, Community Based Organizations, and the business community.
- Develop a communication strategy to be used internally with other City of Yorkton departments as it pertains to assisting individuals presenting in city facilities with substance use or mental health challenges.
- Maintain accurate records and documentation in accordance with evaluation and funding agreement standards.
- Other duties as assigned.
Qualifications
- Grade 12 or Equivalent
- 2 years of related experience in an outreach capacity.
- Experience working with individuals living with mental health and substance use disorders would be an asset.
- Must maintain a clear RCMP criminal record check.
Hours of Work:
This is a full time term position ending on March 31, 2026. Shifts will be scheduled Monday to Friday, however, some schedule adaptations may be required to attend various events.
