Muskeg Lake Cree Nations Health and Wellness Department is seeking a Licensed Practical Nurse(LPN)  Home Care, who is passionate about providing comprehensive, culturally safe, and client-centered nursing care to community members within their homes and in the community.  Under the direction of the Director of Health & Wellness, the LPN will assess, plan, implement, and evaluate nursing care based on individual client needs, while respecting community values and traditions.

ROLES & RESPONSIBILITIES:

    Provide professional nursing care within the LPN scope of practice as per provincial regulations and that which meets existing standards for home care;
    Conduct regular home visits for assessment, medication administration, wound care, chronic disease management, personal care, and health monitoring;
    Develop and maintain individualized care plans in collaboration with clients, families, and interdisciplinary health care team;
    Assist with activities of daily living (ADLs) where required;
    Monitor and report changes in clients' health status where required;
    Develop and maintain a close working relationship with Muskeg Lake Cree Nation staff, and community members;
    Input, design and implement home care programs as directed by community needs;
    Provide home care program statistical reports as required; and
    Provide support to auxiliary health staff.
