With direction from Willow Cree Health Services (WCHS), the incumbent will support and adhere to the Mission/Vision of the organization to provide services to the community of Beardys & Okemasis Cree Nation (BOCN). The Home & Community Care (HCC) Coordinator is responsible for providing and participating in the delivery of a high standard of home & community health. The HCC-Coordinator is responsible for providing, participating and directing staff in the delivery of a high standard of community health.  The HCC-Coordinator provides assessments for community band members needing assistance at home.  The services provided are client and family focused including initial assessment, one on one teaching, treatment, and rehabilitation to prevent deterioration and promote independence. The focus is on preventative health care initiatives and health promotion programming.

QUALIFICATIONS
Education
    Bachelor of Nursing degree and in good standing with SRNA
Experience
    Minimum of three years supervisory experience
    Experience in completing annual work plans and reporting is an asset
    Working with First Nations is considered an asset
General Requirements
    Able to maintain strict confidentiality
    Have an excellent rapport with people
    Strong organizational, communication, and time management skills
    Resourceful; knowledge of the services available to community band members
    Valid Saskatchewan Class 5 Drivers Licence
