Job Description: This position works as part of an interdisciplinary primary health care team. Under the direction of the Primary Health Care Manager, the Nurse Practitioner will provide primary health care services to the community. The Nurse Practitioner will be responsible for assessment, planning, implementation and evaluation of care for clients in the community both independently and collaboratively with a physician, depending upon the complexity of client needs. This includes: health promotion and protection; first line treatment and emergency services; provision of supportive counselling and instruction in management of medical conditions; disease and injury prevention; initiating and conducting community needs assessments and participating in evaluative projects; and collaboration with and referral to other health care professionals. This is a community-based position therefore the incumbent is required to work at a high level of independent clinical practice.
This position will be part of the Intermediate Care Service within the Primary Health Care Networks. This service addresses a provincial health system service gap, where some seniors (adults aged 65+) with multiple complex health issues have difficulty in accessing an office based primary care provider.
