As a member of the Home and Community Care Team, the LPN will effectively utilize their educational, observational, analytical, and organizational skills to assist the RN in assessing home care clients. The LPN will contribute to the development of an appropriate plan of care in agreement with the client to successfully manage client needs. The LPN will assist clients and families to coordinate care, so clients are able to attain their optimum level of independent function in the activities of daily living.
Qualifications:
Graduate of approved Licensed Practical Nursing program
Current registration with the Saskatchewan Association of Licensed Practical Nurses (SALPN).
Minimum of 2 years nursing experience in community health, long-term care or home care
Current CPR Level C
Clean CPC and Vulnerable Sector Check
A valid class 5 Sask. Drivers License
Knowledge, Skills, and Abilities:
Ability to work independently and as a member of the multidisciplinary team
Excellent verbal and written communication skills
Ability to organize workload demands utilizing time management and organizational skills
Demonstrated knowledge, respect, and understanding of First Nations culture and traditions.
