Job Description: The Senior Assessor Coordinator is a supervisory, team leader position in Primary Health Care, reporting directly to a CPAS Manager. The position provides clinical supervision to the team/staff within the department in the functions of case management practice. The position requires the use of high order knowledge, clinical expertise and strong interpersonal skills to coach, mentor, orientate, and provide education to the team members. Drawing on standards of practice for case management; professional codes of ethics and standards of practice in health care; program policy and procedures; and relevant legislation, the incumbent will ensure that the team provides high quality client service and professional standards of practice are maintained.
The assignment of time may include:
-Supervising complex clinical cases , supporting, coaching, mentoring, and orientating members to assigned area
-Assisting with the navigation of Alternate Level OF Care (ALC) clients
-Involved in development, implementation and evaluation of projects related to case management practice
Required Qualifications
Recognized degree in Social Work, Nursing, Occupational Therapy, or Physical Therapy
Registered or eligible for registration with the appropriate licensing body
Valid Saskatchewan driver's license and access to a reliable vehicle
Experience
Minimum five years related clinical experience with demonstrated leadership ability. Two years in the most recent 4 year period need to include CPAS experience.
